Chest pain--another adverse reaction to vancomycin.

چکیده
The article by John Cappola et al (Chest 1989; 95:1309-15) states that randomized studies assessing the efficacy of coronary artery bypass surgery in acute myocardial infarction are not available. We wish to draw their attention to our article, “Urgent Surgical Reperfusion in Acute Evolving Myocardial Infarction; a Randomized Controlled Study” published in Circulation 1988; 79(3):I17178. This work was performed at the University of Ottawa Heart Institute, Ottawa Civic Hospital, comparing the standard medical management of acute evolving myocardial infarction and aortocoronary bypass graft surgery. Sixty-eight patients presenting within 4 h of the onset of chest pain were randomized into medical and surgical groups. Our conclusions were that urgent surgical reperfusion in acute evolving myocardial infarction was safe and effective and appeared to reduce early and late mortality. However, at three months post infarction there was no significant differences in the ejection fraction in the survivors between the two groups.

Chest pain and high troponin level without significant respiratory symptoms in young patients with COVID-19
Background: Of all patients infected with COVID-19, 95% have mild symptoms, but 5% may experience severe illness. There are reports of myocardial injury associated with the COVID19 infection in middle aged and old people with baseline cardiac conditions. Acute myocardial injury has been suggested as a marker for disease severity. Sometimes it is hard to differentiate between acute coronary synd...
متن کاملAnother cause of chest pain: Staphylococcus aureus sternal osteomyelitis in an otherwise healthy adult
Chest pain requires a detailed differential diagnosis with good history-taking skills to differentiate between cardiogenic and noncardiogenic causes. Moreover, when other symptoms such as fever and elevated white blood cell count are involved, it may be necessary to consider causes that include infectious sources. A 53-year-old female with no significant past medical history returned to the hos...
متن کاملChest Pain? An Unusual Presentation of Vertebral Osteomyelitis
We present a case of vertebral osteomyelitis presenting as chest pain. The patient initially underwent a CT chest angiogram to rule out a pulmonary embolism, which incidentally showed a soft tissue vertebral mass at T3-T4 disk space. Subsequent thoracic vertebral MRI was consistent with osteomyelitis with cord compression. Tissue culture from a CT-guided biopsy grew MRSA. The patient was succes...
متن کاملAdverse drug reaction Medication-induced oesophageal injury leading to broncho-oesophageal fistula
Medication-induced oesophageal injury is one of the least recognised side-eVects of oral medication and, in contrast to other oesophageal pathologies, is rarely considered in the diVerential diagnosis of chest pain. We describe a case of medicationinduced oesophageal injury with a rare complication in which the diagnosis was not considered until the characteristic features were demonstrated at ...
متن کاملPredictors of Adverse Outcomes of Patients with Chest Pain and Primary Diagnosis of Non-Cardiac Pain at the Time of Discharge from Emergency Department: A 30-Days Prospective Study
BACKGROUND Chest pain is a common symptom for referring patients to emergency departments (ED). Among those referred, some are admitted to hospitals with a definite or tentative diagnosis of acute coronary syndrome and some are discharged with primary diagnosis of non-cardiac chest pain. This study aimed at investigating 30 days' adverse outcomes of patients discharged from ED of a major heart ...
